3. Health Disclosure and Service Modifications The Services provided are for cosmetic purposes only. Practitioners are not medical doctors and do not provide medical diagnosis.

• Collaboration with Medical Teams: Oncology-informed services are intended to complement, not replace, clinical care. Clients are encouraged to consult their primary medical team prior to sessions.

• Information Accuracy: You agree to provide accurate information regarding your health history, medications, and surface sensitivities. Certain factors may require service modifications or the postponement of a session.

• Service Discretion: The Company reserves the right to modify or decline a Service if it is deemed inappropriate for the esthetic scope or potentially disruptive to skin surface integrity.

4. Client Consent and Assumption of Risk Prior to your session, you will be required to complete intake and choice centered consent forms.

• Insurance: Documentation provided (such as service summaries) is for personal records only. We do not guarantee reimbursement from third-party payers.

• Acknowledgment: By proceeding, you acknowledge that results vary, no specific outcome is guaranteed, and you voluntarily assume the risks inherent in professional skin care and electrolysis.

This provision does not prevent either party from seeking relief in small claims court where applicable. Prior to arbitration, both parties agree to attempt good faith informal resolution.
